2012 Player Medical Release

NCAA Rules for Recruiting

The following is important so please follow the instructions:

For tryouts you need to bring with you and your daughter   

    2012 SRVA/USAV REGISTRATION FORM, completed

    2012 SRVA/USAV MEDICAL RELEASE FORM, completed

    2012 Player Info Form, if they did not give it to us at the October 16th meeting.

    $30 in cash or check, made payable to CCCVBC, unless you have prepaid by Thursday, October 27, 2011

The Medical Release Form can be found under FORMS, it does not have to be notarized in either the states of Alabama, Georgia or Tennessee.

Everyone, even if you have already printed off a 2012 USA Volleyball Membership form from our website,  needs to do the following:

    1. Go to www.srva.org

    2. Click on SRVA ONLINE

    3. Click on SRVA ONLINE FOR INDIVIDUAL REGISTRATION

        Then!!!!

If you were on one of our teams last year OR if you were on any Southern Region team last year OR if you tried out for a Southern Region team last, ours or another but did not make a team.  Do the following….

a. Enter your daughter’s last name and date of birth, click find

b. If it populates your information is in the system. 

c. It will ask you for a password, if you forgot it, click the FORGOT icon, if you know it enter it and click the SELECT icon.

d. HOPEFULLY another page will open and populate. 

e. Review, correct if necessary and complete the information.  Make sure you enter your daughter’s grade level, height and hand.  Just below is INDIVIDUAL MEMBER CATEGORY, drop down and enter JUNIOR FEMALE PLAYER.  As the parent/guardian, make sure to enter the information required for you too.

f. Click UPDATE

g. You will either get a bunch of red highlighted areas that require some sort of information or the ability to print.  Print the form, and be sure both you and your player have signed the form.

If you NEVER entered information on this site before, then after clicking on SRVA ONLINE, find the icon entitled ADD NEW INDV, click on that and it will     take you to a screen to be completed.  Complete the information, refer to item e  above and print off the registration form.
